The EMR (Electronic Medical Record) is where all clinical documentation for a patient is stored. The EMR allows a provider to open a patient record to both see past documentation, graph out lab results, view scanned documents. This is also where entire visits for appointments will be documented, medications will be prescribed from, and much more.

The History chart tab is used to capture a patient's past medical, family and social history. It is a "historical" chart tab where data remains as last edited from encounter to encounter.
The Problem List chart tab is used to capture a list of conditions a patient has been diagnosed with. This list of problems is used to facilitate workflows throughout the application including the Superbill and Order Entry chart tabs. The Problem List chart tab will also double as the assessment for many users by adding notes to a problem via the edit window.

Templates are used in multiple chart tabs to document the patient's visit efficiently. These templates are typically setup by your implementation team but can be altered per request through the support portal. Templates are used in the following chart tabs: History, Assessment, Review of Systems, HPI, Exams, Orders (and Order Fulfillment), Treatment Plan, and Intake (if enabled).
