CQMsolutions Quality Reporting

iSalus Healthcare's electronic clinical quality measure, hereafter referred to as eCQM, solution is called CQMsolutions. In order to setup eCQM quality reporting for your practice, follow these steps: 

1. Email iSalus Support and request to implement CQMsolutions for your practice. Be sure to specify whether you are reporting eCQM for MIPS and/or Meaningful Use.
2. Setup your practice's MU Eligible Providers in the Meaningful Use Dashboard. 
3. Setup your practice's MIPS Eligible Clinicians in the MIPS Dashboard.
4. Create a user "cqmreports" with no roles. Send your iSalus Support representative the username and password (in 2 separate emails) for this user. Our web services will use this user to pull your data from your database into CQMsolutions.
5. Send your practice's TIN and list of Providers with their NPIs to your support representative. iSalus Support will work with your practice administrator  to complete the setup of CQMsolutions for your practice.
6. Set the Company Setting: Reporting Year to the current reporting period year (e.g. 2022). 

Follow these steps to launch and login to CQMsolutions:

1. Click on the Reports portal.
   
2. Select the CQMsolutions option under the Meaningful Use section.
   
3. Enter your CQMsolutions login credentials and press the Log In button.
   
4. The Home Screen will load.
   

Follow these steps to create a Quality Report: 

1. Click the Create Report menu. 
   
2. Enter a report Description and select the appropriate Begin Date, End Date and CQM Update Year. 
   
3. Check the Providers to run the quality report for. 
   
4. Press the Queue Report button. 
   
5. The report will display on the Home Screen - Queued Report screen. It will run automatically.
   
6. Once the report has completed, click on the Results button to view the report details.
   
7. The quality report displays.
   

1. Login to CQMsolutions.
2. Queued and completed reports display on the home screen. Click on the Results button to view a completed report's details.
   
3. Click on View Detail to view patient level details for the measure. 
   
4. Select a Patient to view the measure logic for the patient.
   
5. The measure logic and patient's data displays. This is helpful for troubleshooting individual patients.
   
6. Click the Patient List button to return to the measure's patient list.
   
7. Select the Dashboard button to return to the list of quality measures.
   

Follow these steps to extract QRDA I, QRDA III and QPP files for submission to Medicare/Medicaid:

1. Login to CQMsolutions.
2. Queued and completed reports display on the home screen. Click on the Results button to view a completed report's details.
   
3. Click on the Detail menu button.
   
4. Select the Downloads button.
   
5. Click Download QRDA III Medicaid or Download QPP MIPS Individual as appropriate. 
   
6. Select the Save As option to specify where the file should be saved.
   
7. Name your report as appropriate and press the Save button. The file is now saved to the specified location on your computer and ready for submission to Medicare/Medicaid.
