Running the Dialysis Survey Report

Running the Dialysis Survey Report requires carefully considering the filter options to produce the level of detail you need the report to contain. If you need to learn how to add this new report to your Connect Report menu then go to this Add a Connect Report link. You will find the report Dialysis Survey Report under the Patient header.

Filters and Fields Defined:

  • Required: Appointment Date Filter: Set an appointment date range - expectation is that this is a monthly survey report run to determine status of incoming patients. Users are cautioned not to run the date range for too lengthy a time as it could cause the report to time out depending on practice patient volume.

    The appointments returned per identified patient are filtered in descending date order (based on the date range and filter options input by the user) and then we return details for the top row (first) appointment listed per patient (prioritizing Provider over Resource appointments). The date and time of the patient's most recent appointment, formatted as MM/DD/YYYY HH.

  • Required: Filter Setting for DM, CKD, and PU as "Problem Required" or "Problem Not Required"- when set to required then the report looks patients that DO HAVE an active problem list item where the specified Problem List item type meets this criteria:
    • DM is any Problem List item with E08 or E10 or E11 
    • CKD is any Problem List item where the associated Diagnosis Code starts with N18
    • PU is any Problem item where the associated Diagnosis Code starts with R80  

      If you set DM, CKD, or PU as "Problem Required" then the patient must meet that criteria to pull into the report, all other patients are ignored. If you don't regularly use the Problem List and want to see more patients on the report then you can opt not to require and of the Problem List filter items.

  • Once patients are identified for the report, we will return general demographic data and next appointment detail, and then this report identifies patient with an On or Off status for the following drug classes: ACE, ARB, SGLT2, MRA, and GLP1 based on the hard coded drug name values listed below. That means IF the drug does not meet our search criteria below then we are not searching for it and it won't be included on this report. It is possible for a patient to return no prescription data if you have not prescription data entered for the patient. Where the patient does have an active prescription for one of these drug classes/specified drug name searches then we provide the brief medication details.
