Adding the Referring/PCP Patients Report

  1. Go to the Reports section, then under EMR select Connect.
  2. Click the icon with a plus sign in the toolbar to create a new Connect report entry.
     


  3. When asked to select the Report, scroll under the Claim header and find Referring/PCP Patients, and enter a description before hitting Next:



  4. Select the desired fields for the report to show, or simply select 'All.' Then hit 'Finish,' then 'Close,' to complete adding the report. The available fields are as follows:

    1. 1y Patient Count: The number of distinct patients who have this provider as their Referring or PCP in their demographics, and who have had appointments in "Checked-In" or "Checked-Out" status in the last year. 
    2. Referring/PCP: The name of the Referring Provider/PCP. 
    3. 6mo Pt Count: The number of distinct patients who have this provider as their Referring or PCP in their demographics, and who have had appointments in "Checked-In" or "Checked-Out" status in the last six months.
    4. 2y Pt Count: The number of distinct patients who have this provider as their Referring or PCP in their demographics, and who have had appointments in "Checked-In" or "Checked-Out" status in the last two years.
