Adding Patient Roster Report

  1. Go to the Reports section, then under EMR select Connect.
  2. Click the icon with a plus sign in the toolbar to create a new Connect report entry.
     


  3. When asked to select the Report, scroll under the Patient header and find Patient Roster. Then fill out the description and click Next:


  4. Select the desired fields for the report to show, or simply select 'All.' Then hit 'Finish,' then 'Close,' to complete adding the report. The available fields are as follows:

    1. Address 1: The first line of the patient's address.
    2. Address 2: The second line of the patient's address.
    3. City: The city from the patient's address. 
    4. Comments: The comments from the patient roster entry. 
    5. Admission Date: The admission date from the patient roster entry.
    6. DOB: The patient's date of birth.
    7. Visit Date: The visit date from the patient roster entry. 
    8. Gender: The gender code of the patient.
    9. Home Phone: The patient's primary "Home Phone" number. 
    10. Other Phone: The patient's primary "Other Phone" number.
    11. Chart #: The chart number from the patient's account. 
    12. Patient First: The first name of the patient. 
    13. Patient Last: The last name of the patient. 
    14. Recurrence: The recurrence set on the patient roster entry.
    15. Referring: The referring provider from the patient roster entry. 
    16. Resource: The resource from the patient roster entry.  
    17. Room: The room set on the patient roster entry. This is a manually keyed field. 
    18. Location: The service location from the patient roster entry.
    19. State: The state from the patient's address.
    20. Work Phone: The patient's primary "Work Phone" number. 
    21. Zip: The zip code from the patient's address.
