Patient CKD Education Status

The new Patient CKD Education Status provides a list of patients based on upcoming appointment date search, providing a status on CKD Education (G0420). Patients will show as Y to indicate yes the patient shows a G0420 history, or a N to indicate not history found for the patient for G0420 history. To be included on the report, a patient must meet these requirements:

1. An active Problem List item related to N18.4 or N18.5 
2. An  eGFR < 30.0
   
   

   
   

1. Go to the Reports section, then under EMR select Connect.
2. Click the icon with a plus sign in the toolbar to create a new Connect report entry.
    
   
   
3. When asked to select the Report, scroll under the Patient section header and find Patient CKD Education Status. 
4. Type a description, input desired parameters and then select Next. 
   
5. Select the desired fields for the report to show, or simply select 'All.' Then hit 'Finish,' then 'Close,' to complete adding the report. The available fields are as follows:
