CCM Days Since Verbal Consent

The CCM Days Since Verbal Consent Report allows a practice to pull a monthly audit list see the last call date and time for patients who have an active case management services for CCM and have a consent date recorded. The practice can designate the number of days since the verbal consent where 0 will pull all patients in the verbal consent date range, but any other number would exclude patients where consent was received in the last __ number of days.

1. Go to the Reports section, then under EMR select Connect.
2. Click the icon with a plus sign in the toolbar to create a new Connect report entry.
    
   
   
3. When asked to select the Report, scroll under the Case Management header and find CCM Days Since Verbal Consent. 
4. Type a description, input desired parameters (including to set the Client), and then select Next. 
5. Select the desired fields for the report to show, or simply select 'All.' Then hit 'Finish,' then 'Close,' to complete adding the report. The available fields are as follows:
   
   

To run this report, perform the following steps:

1. If you just created the CCM Missing Consent Dates Connect report, click the refresh icon on the toolbar:
   
   
   
2. Select the newly created report in the list.
3. Click the drop-down next to the hammer and wrench icon on the toolbar. Then click Run Report.
